JJDA COMPANY REGISTRATION AGREEMENT
Waiver

I hereby waive any liability against The Jill Justin Dance Alliance on behalf of my child for any personal injury
or loss of property while participating in any activity or any class offered by the school. In case of emergency
illness or injury in which I cannot be present or reached, I authorize The Jill Justin Dance Alliance staff to seek
the necessary medical attention for my child.

In addition, I give permission for my child to be photographed or videotaped while dancing. I understand that
the photographs and video will be used by The Jill Justin Dance Alliance for the purpose of publicizing and
promoting the studio’s programs and services and may be included on websites featuring the studio.
Compensation will not be expected.

Acknowledgement of Company Policies

Parent Guidelines: I have read and understand the guidelines for JJDA parents at the studio and at
competitions and events.
​
Tuition: Tuition is due no later than the 15th of each month. After the 15th of the month a $15 late fee will
apply.
​
Private Lessons: The fee for private lessons is $30 per half hour. If your child is asked to perform a solo or
needs a private lesson for any reason, the $30 fee will be due at the time of the lesson and is payable directly to
the instructor. All Company members competing in Jazz, Lyrical, Contemporary or Musical Theater must do
one ballet private per month.

Attendance: Students may only miss one of each class per month. Company members with poor attendance
will not be permitted to participate in small groups, solos or duos and may be dismissed from the Company.

Uniforms: All JJDA classes have a dress code and require the correct shoes. Students should have hair bands
with them at all times so that they can put their hair up when needed.

Important Dates: I have reviewed the Competition and Workshop schedule. I understand that all competitions
and rehearsals are mandatory unless otherwise indicated.

Company/Choreography Fees: The Company Fee is $125 per dancer. This fee is used for teacher expenses at
competition, props and spirit week expenses. Any dance by an outside choreographer will have a choreography
fee of $125 per dancer for each dance.

Competition/Workshop Fees: I have reviewed the due dates for all fees and I understand that my child will
not be entered in competitions or workshops if fees are not paid on time.
​
Costumes: We recommend that families budget $125 per costume. Some costumes may be more and some will
be less. Solo costumes are generally one of a kind and are more expensive. Costume payments are due on
October 1st, November 1st, December 1st and February 1st. Costumes will not be distributed unless they are paid
in full. All other fees must be paid in full at that time as well. Specific jewelry and makeup are required and
are an additional cost.

Fundraising: The school offers optional fundraisers. Money raised through these activities is the property of
JJDA. Families may apply these funds to the cost of competitions, costumes and other company activities. If
dancers leave the company they forfeit any funds remaining in their account.
​
Cash Awards: Cash awards won by group dances at competition will be applied to company expenses such as
rehearsal space rental. Solo and duo cash awards will be applied to students’ fundraising accounts.
​
Recital: All tuition and fees must be paid in full prior to the recital. Dancers with an outstanding balance will
not be permitted to dance in the recital.
​
Commitment: I understand that my child is committing to the JJDA Company for the entire season, which
ends after the recital and/or Nationals next summer. Students who do not complete the year will not be
reimbursed for any fees paid, including but not limited to, competition fees, costume fees, choreography fees
and tuition. I understand that I will be responsible for any outstanding fees due at that time and will not receive
reimbursement for any fees previously paid.
